2017 SUMMARY OF TOTAL REWARDS FOR FULL-TIME EMPLOYEES

KAISER PERMANENTE HEALTH INSURANCE HMO & HRA Health Reimbursement Account & E-Flex HRA debit card

Open enrollment is annual December 1 – 31 for January enrollment. Available to full-time employees on 1st day of the month following 30 days of employment.  The Company pays 100% of employee premium. After 3rd year Company pays 25% dependents premium, after 4th year Company pays 50% dependents premium, after 5th year Company pays 100% dependents premium. The Company also pays the first $1,200 of $2,500 deductible for employee and first $2,400 of the $5,000 deductible per family (if applicable) via E-Flex HRA debit card. Newly hired employees are subject to a prorated amount on their E-Flex HRA debit cards depending on hire and eligibility date. Mobile App available.

GUARDIAN DENTAL INSURANCE    

Open enrollment is December 1 through December 31 for January enrollment. Available to full-time employees & eligible dependents on 1st day of the month following 30 days of employment.  After 30 days Company pays 90% of employee premium, after 3rd year Company pays 25% dependents premium, after 4th year Company pays 50% dependents premium, after 5th year Company pays 90% dependents premium. GUARDIAN LIFE & LONG-TERM DISABILITY INSURANCE        

$25,000 Basic Life Insurance policy & $25,000 Accidental Death & Dismemberment and Long-Term Disability (in excess of 30 days and up to 2 years) 66 2/3% of your gross monthly pay up to $3,500 per month integrated with State Disability Insurance (SDI) and Workers’ Compensation benefits. Age rated coverage reductions after 65 years of age. The Company pays 100% of premium on behalf of full-time employees on 1st day of the month following 30 days of employment.

OPTIONAL GUARDIAN LIFE INSURANCE        

Available to full-time employees & eligible dependents on 1st day of the month following 30 days of employment.  Employee pays premium by automatic payroll deduction for up to $200,000 employee & $100,000 dependent coverage available at our Corporate-discounted rate. Age rated coverage reductions after 65 years of age.

PAYROLL CHECK DIRECT DEPOSIT

Any amount of your payroll check each pay day can be automatically deposited to the financial institution of your choice.

PAYROLL DIRECT DEPOSIT TO ADP DEBIT CARD

ADP Direct Deposit Debit cards with no monthly service fee are available to all employees. Mobile App available.

FLEXIBLE BENEFITS SPENDING ACCOUNTS – FBSA/FSA                  

Pre-Tax monthly payroll deductions available to full-time employees who enroll prior to December 1 each year.  Full funds are available January 1 of each year. Mobile App available.

Health Care Spending Account – Reimbursement of up to $2,600 for out-of-pocket expenses for medical, dental, and vision care expenses for employees and dependents

Dependent Care Spending Account – Reimbursement of up to $5,000 for daycare expenses for disabled dependents, dependent children or parents of employees

401(k) RETIREMENT PLAN     

Upon employment all employees (21 years of age and older) will automatically be enrolled with minimum 3% pre-tax contribution; employees may voluntarily elect to increase that contribution to 90% (or maximum amount allowable $18,000 under 2016 Federal guidelines) of pay or opt out of the plan completely; Mobile App available. Company matches up to $1,200 per year. Vesting in Matching Contributions and any Non-elective Contributions:

FLOATING HOLIDAY – Full-time Ambulance Division

Each Ambulance Division employee will be granted the right to designate a floating holiday for holiday pay purposes only (subject to management approval).  It is not intended to allow for a day off but allows for one additional holiday pay day and applies to only regularly scheduled shifts or trade days.

EMPLOYEE ASSISTANCE PROGRAM    AVANTE BEHAVIORAL HEALTH   

The Company pays 100% of the premium for all employees and their eligible dependents. All employees and their immediate family are eligible for up to 6 clinical consultations each calendar year with mental health professionals for issues related to Stress Management, Anger Management, Substance Abuse, Depression & Persistent Anxiety, Battered Spouse & Rape, Interpersonal Relationships, Work Related Issues, Grief/Loss, Crisis Situations and Workplace Transition. They also provide two 30-minute telephonic consultations related to Legal Assistance, Financial Assistance, Identity Theft, Tax Planning, Retirement and Estate Planning.

Most full-time employees are eligible.  Forms must be completed in advance by the employee (available in Human Resources) and noted on the application at the time the referred candidate applies for a position.
